How to raise claim requests for Health Insurance policies
Follow these steps to raise Health Insurance claim requests with Bajaj Allianz General Insurance.

Step 1 - Locate a network hospital near you

Step 2 - Verify patient’s identity
Share the patient's policy number and show your Bajaj Allianz cashless card along with valid identity proof (PAN, voter ID, passport) at the hospital.

Step 3 - Submit the pre-authorisation form
Fill out and submit the pre-authorisation form at the hospital. The hospital will send this form to the Bajaj Allianz – Health Administration Team (HAT).

Step 4 - Claim settlement
Once the verification is complete, the insurer will give the final confirmation within one working day and settle the claim directly with the hospital. The claim will be processed per the policy's terms and conditions.
Someone from Bajaj Allianz General Insurance will contact the healthcare provider for any additional information required. The hospital needs to send discharge summary, bills, and prescriptions on the day of discharge. Upon receiving all the required documents, the insurer will send an authorisation letter within 7 working days.
After clearing all the formalities, Bajaj General Insurance will settle the bills directly with the hospital.
For reimbursement claims:
1. Fill the reimbursement claim form online on the Bajaj Allianz website by entering your health ID card number. You need to file the claim form within 30 days after getting discharged from the hospital.
2. Collect and submit the necessary documents for claim settlement, such as claim form, hospital discharge summary, bills and prescriptions to the Bajaj Allianz – Health Administration Team (HAT).
3. Once the verification is complete, the insurer will give the final confirmation. The claim will be processed as per the terms and conditions of the policy.
In case of any queries, Bajaj Allianz team will contact you for additional information. Upon receiving all the required documents, the insurer will settle your claim within 10 working days.

Take your vehicle or have it towed for repair to the nearest garage. You can also choose any of the insurer's network garages for availing of a cashless facility.
